​Instructions

  • After you print the petition, fill in the blank field in the first paragraph with the state name.
  • Only residents from one state can sign on a single page. If you have individuals from multiple states, you must have a separate page for each state.
  • Confirm the individual signing is 18 or older. 
  • Ensure that all fields have been completely filled out. Incomplete fields invalidate that signature.

    • City
    • Zip Code
    • Date
    • Signature
  • When the page is filled, or once a month if you are planning to fill more than one page, please mail the completed pages to the address on the bottom of the petition.
